FSR-2090-0491 — Mis-Shipped Cultivar Recovery
Recovery of a GCL-2 GreenWorld cultivar (UF-29047) erroneously delivered to a public civic annex under a mislabeled logistics manifest. Specimen recovered intact and returned to containment with full chain-of-custody; no public exposure.
Engagement classification. GCL-2 recovery in a public setting following a logistics mislabel. Two-person rule applied (one handler, one custody officer) and chain-of-custody mandatory per VC-SOP-0610. Cultivar carries a mild contact-irritant hazard; field-grade gloves and eye protection required. Not a breach — controlled recovery.
1. Site & Conditions
| Field | Entry |
|---|---|
| Site name / client | Civic Botanical Annex (municipal public display, non-partner) |
| Location / access | Public conservatory wing; specimen held in receiving dock, not yet placed on display. Staff cooperative. |
| Engagement type | Recovery |
| Ambient temp / RH | 20 °C / 60 %RH; dock climate-controlled |
| Conditions on arrival | Specimen still in original GreenWorld transport crate, unopened; manifest mislabeled as a benign ornamental donation. No handling by annex staff confirmed. |
| Public proximity | Receiving dock cordoned; no public access during recovery window. |
2. Specimen Identification
| Field | Entry |
|---|---|
| Common name | Caustic Bract Lily (engineered contact-irritant cultivar) |
| Cultivar / accession | UF-29047 — vault stock, diverted by manifest swap with lot UF-29074 |
| Assessed GCL grade | GCL-2 (mild contact-irritant sap; no aerial dispersal, no predation) |
| Approx. size | ~0.7 m potted specimen, single rosette, est. 4 kg with pot |
| Observed hazards | Irritant latex in bract tissue; VOC/Toxin Index low but non-zero (see §4) |
| Provenance | Confirmed — genetic match to vault lot via field PhytoSense swab; crate seal intact |
3. Work Performed
- Verified the crate seal and label discrepancy against the dispatch manifest; confirmed transposition of accession IDs
UF-29047↔UF-29074at the logistics hub. - Established a dock cordon; briefed two-person roles: Mallow handler, Sorrel custody officer logging each transfer step.
- Donned field-grade gloves, eye protection, and dosimeter per VC-SOP-0301 GCL-2 subset. Confirmed PhytoSense handset calibration (cal age 4 d).
- Captured arrival readings (Section 4) without opening the crate; VOC/Toxin Index sampled at the crate vent.
- Opened the crate under the cordon, confirmed specimen identity and intact condition, and re-secured it in a GCL-2 sealed transport sleeve.
- Initiated chain-of-custody record
COC-2090-0491per VC-SOP-0610; Sorrel co-signed each handoff from dock to transport pod. - Recovered the mislabeled manifest copy and crate as evidence for the logistics audit; performed field decon on tools and gloves.
- Confirmed dosimeter clear; broke cordon; transported to GreenWorld intake for re-containment.

5. Parts & Consumables
| Item | Part / Lot | Qty | Notes |
|---|---|---|---|
| GCL-2 sealed transport sleeve | TS2-0440 | 1 | Potted specimen re-secured |
| GCL-2 sealed transport pod | TP2-0188 | 1 | Transit to intake |

| Chain-of-custody seal tags | COC-SEAL-22 | 4 | Numbered, logged to COC-2090-0491 |
6. Containment Actions
- PPE: field-grade gloves, eye protection, dosimeter (GCL-2 subset)
- Containment method: sealed transport sleeve + pod; crate seal preserved as evidence
- Two-person rule observed: Yes — handler + custody officer
- Field dispersal control: crate vent sampled before opening; opened only under cordon
- Chain-of-custody: COC-2090-0491 opened and co-signed at every transfer per VC-SOP-0610
- Decon on egress: tools and gloves field-deconned; dosimeter clear
7. Outcome & Disposition
| Outcome | Recovered |
| Specimen disposition | Returned to GCL-2 Bio-Vault array; re-intake under accession UF-29047; chain-of-custody COC-2090-0491 closed at intake co-sign. |
| Site left in condition | Dock cleared; annex staff briefed; no specimen residue. Annex confirmed no prior handling. |
| Emergency procedure triggered? | No — controlled recovery, no breach declared. |
| Recovery duration | 1.6 h on-site (within 2.0 h dispatch target) |
8. Follow-Up Actions
- Logistics audit of manifest transposition at the hub — owner: S. Aralia — due 2090-04-25. Done
- Intake re-containment confirmation and re-catalog — owner: D. Hemlock — due 2090-04-23. Done
- Corrective action: add accession-ID barcode verification at outbound dock — owner: Logistics Lead — due 2090-05-15.
9. Lessons Learned
Lesson — manifest mislabeling is a containment failure mode, not just a logistics error. A single transposed accession ID (
UF-29047 ↔ UF-29074) placed a GCL-2 irritant cultivar into a public annex. The intact crate seal and prompt recovery prevented any exposure, but the root cause was an unverified outbound manifest. Recommendation: mandate scan-verified accession barcodes at the outbound dock and a destination-grade check that blocks GCL-2+ shipments to non-partner addresses.
